Six items:
1-Patient-centered Care
Definition: Recognize the patient or designee as the source of control and full partner in providing compassionate and coordinated care based on respect for patient’s preferences, values, and needs.
2-Teamwork and Collaboration
Definition: Function effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care.
3-Evidence-based Practice (EBP)
Definition: Integrate best current evidence with clinical expertise and patient/family preferences and values for delivery of optimal health care.
4-Safety
Definition: Minimizes risk of harm to patients and providers through both system effectiveness and individual performance.
5-Informatics
Definition:Use information and technology to communicate, manage knowledge, mitigate error, and support decision making.
6-Quality Improvement (QI)
Definition: Use data to monitor the outcomes of care processes and use improvement methods to design and test changes to continuously improve the quality and safety of health care systems.
Part A
Outline, just list:
1) the topic title;
2)the quality of care or safety issue in one concise sentence;
3) your one strategy for resolving the concern.
Part B
The paper is to be no longer than two pages, double-spaced with at least 2 references, not from your textbook.. This involves critical thinking and the ability to convey your beliefs, values, or concerns in a retrospective review of observations, facts, readings, or impressions.
-Title
-Identification of the quality of care or safety issue. Explain why the choice by giving the rationale.
-Reflect on when you became aware that it is an issue/concern and under what circumstances have you mused about this item.
-Indicate how you would suggest the issue/concern be resolved; i.e., at least one strategy.
